Triage

We operate a triage system at the practice which means that your doctor may call you to to speak to you about your symptoms before booking you an appointment. In some cases medication can be prescribed to you without the need for you to come into the surgery.

Mental Health

Mental health services are offered free at the practice, but in some cases you’ll need a referral from your GP, there are many self help programmes and groups that are available which you can refer your self to, visit us to see what support is available to you.

Travel Vaccinations

You may require certain vaccinations when travelling abroad.

IDEALLY WE NEED 8 WEEKS NOTICE TO BOOK YOUR VACCINATION APPOINTMENT

If your required vaccine in not on our list you will need to attend a Travel Clinic where they specialise in travel vaccines. Local travel clinics can be found by searching the internet.

If you wish to book in for any of the vaccinations we do administer, please click this link https://florey.accurx.com/p/P89008 and complete an Admin Query Form to request a Travel Risk Assessment Form. Please note we require one form to be completed by each traveller.

Here’s how to complete the Admin query form:-

Select the ‘Something else’ option.

Free type in the box ‘Travel vaccine request form

Continue to complete this form with your personal information and click Submit.

We will respond by text, sending you a link to our Travel Vaccine Risk Assessment Form, which you are required to complete before we can offer any appointment.

We require one form to be completed for every person travelling and ideally need 8 weeks notice for your appointment.

When we receive your completed Risk Assessment Form a member of the team will contact you regarding the next steps.

Diabetic

Counselling is a therapy which involves talking through your problems to a counsellor.

COPD

Chronic Disease Monitoring follows the progress of a collection of lung diseases including chronic bronchitis, emphysema and chronic obstructive airways disease.

Coronary Heart Disease

Coronary heart disease (CHD) is the term that describes what happens when your heart’s blood supply is blocked or interrupted by a build-up of fatty substances in the coronary arteries.

If you have been diagnosed CHD you’ll require regular appointments with your doctor.

Anticoagulations Monitoring

Anticoagulants are medicines that help prevent blood clots. They’re given to people at a high risk of getting clots, to reduce their chances of developing serious conditions such as strokes and heart attacks.

Anticoagulation medicines must be monitored. If you’re prescribed anticoagulants you should book regular appointments with your doctor for monitoring.
